The Roth IRA is a government-sponsored, after-tax
retirement plan for individuals that accepts contributions of earned
income annually. Earnings in this account are generally tax-free.
If your account representative charges certain
fees that you wish to have deducted from your account, you will need
to complete the Authorization for Payment of Management/Advisory
Fees.
If you wish to have your cash balance invested
automatically, you'll want to complete the Standing Purchase Instructions
form.
If you are including a check with your application,
you may wish to complete the Investment Authorization and Contribution
Form as well.
If you are moving your Roth IRA from another
custodian, you will need to complete the Transfer Form. Roth IRAs
cannot accept rollover funds from employer plans.
