Traditional IRA Documents and Forms

The Traditional IRA is a government-sponsored retirement plan for individuals that accepts contributions of earned income annually or funds from a former employer's retirement plan. It is generally used for pre-tax monies.

To establish a Traditional IRA, you will need to complete the establishment documents below.

If your account representative charges certain fees that you wish to have deducted from your account, you will need to complete the Authorization for Payment of Management/Advisory Fees.

If you wish to have your cash balance invested automatically, you'll want to complete the Standing Purchase Instructions form.

If you are including a check with your application, you may wish to complete the Investment Authorization and Contribution Form as well (please use the second page of the Contribution Form, called "Rollover Certification," if you are moving employer funds).

If you are moving your account from another custodian or from an employer's plan, you will need to complete the Transfer Form.
